Your privacy is very important to us. We collect personal information for the purposes of pastoral care and relevant communication for Macquarie Life Church ministries and activities.

Access to your information is only available to staff and certain registered volunteers at Macquarie Life Church where is needed for their role. Information is secured on our data storage systems following industry best practise. Staff and registered volunteers will treat personal information with great respect and in keeping with our privacy policy.

I give permission for Macquarie Life Church authorised staff and/or volunteers to obtain medical, hospital or ambulance assistance at any time they consider it necessary. I understand that every effort will be made to contact me or any emergency contact list before instituting such procedures. I acknowledge that I will be liable for any and all medical/hospital/ambulance costs and expenses incurred in my child's treatment.

Macquarie Life Church staff and/or volunteers will provide supervision at check-in and pick-up points at this venue. No child will be allowed to enter or leave the venue until they are registered by a parent/guardian or picked up by a parent/guardian. A person authorized by you may by able to check-in and/or pick up your child if prior consent is arranged with authorised staff and/or volunteers. All children are supervised within the Macquarie Kids ministry program areas with appropriate age-related ratios. I accept the supervision policy.

I agree to indemnity, release and hold blameless Macquarie Life Church, it's employees and volunteers against any and all claims arising from, or in connection with injury, accident, misfortune, damage or loss that may occur to my child, and/or my/their property, equipment or personal effects while present at Macquarie Life Church, including any injury my child may cause to another person or their property.

I give consent for my child to participate in activities they may choose while while attending Macquarie Life Church. I agree and understand that staff and volunteers of Macquarie Life Church reserves the right to exercise its discretion to refuse to register any person upon medical and/or other grounds, without providing a detailed reason for doing so.

I give consent for my child to be captured via photographic, video and audio media at Macquarie Life Church. Macquarie Life Church reserves the right to use this material for promotional purposes. it is a condition of entry that all claims are waived in relation to inclusion of my child's likeness in such films and/or recordings.

I allow Macquarie Kids ministry to utilise my personal information to notify us of news at Macquarie Life Church and provide it to third parties within the church organisation to help provide services to you.
